At St Vincents & Mercy Private Hospital, we are committed to providing you with the best possible patient care before you arrive, during your stay and after you leave our hospital.

To help us prepare for your admission we require your admission details form prior to the day of your admission. This form should be sent to our Booking Office at 59-61 Victoria Parade Fitzroy 3065 or faxed through on 03 9419 6585, as soon as possible.
