Continuum of care

The Continuum of Care department provides an integrated approach to patient management. It incorporates preadmission clinics, home health liaison, discharge planning, Department of Veterans Affairs co-ordination and a social worker.

Discharge planning may include:

• home nursing
• organising Council services such as 'Meals on Wheels' or home cleaning,
• occupational therapy referrals for aids to be installed at home,
• transfer and referrals to "after care" or formal rehabilitation programs if requested by your specialist as essential for your recuperation,
• outpatient physiotherapy services.

The Continuum of Care team work closely with medical and nursing staff on the wards to make sure you receive the best care from when you are admitted to when you are discharged.
