The Home Health Liaison Nurse is based at each campus and on referral, is able to arrange post-discharge services to support patient/families in the community. Home Health Liaison will make an assessment whilst you are in hospital and discuss your needs with you to provide the best possible support for you at home.
Supported services and eligibility is dependent on your health fund policy and/or home and community care criteria. Providing you meet the appropriate criteria you may be eligible for the following:
* hospital in the home services: available on referral by your doctor and provides care that would normally be available in the hospital setting and includes the following; intravenous medication, complicated wound care and drain tube management;
* post acute management: this includes removal of staples, wound care, stoma care, medication management, continence management, diabetes care and palliative care
* council services: this includes assistance with hygiene, home help, meals on wheels and respite care
* convalescent/respite care: available to patients who are appropriate for discharge, however require minimal assistance with activities of daily living in a supported residence
